Update dart_mcp_server README.md file

This file is mostly auto-generated, to update it run the script under pkgs/dart_mcp_server at the path tool/update_readme.dart. The working directory must be set as pkgs/dart_mcp_server when running the script.

This needs to be ran any time any tools are updated, added, or removed.
